Plan Options

Both HSA 100^SM Plan and HSA 80^SM Plan combine high deductible medical insurance with a tax-favored savings account. To be eligible, you must be covered under a qualified health plan and not covered by Medicare.

The plans cover the following at either 100%  (HSA 100^SM Plan) ir 80% (HSA 80^SM Plan) after the deductible is met:

• Includes Network or Non-Network Coverage
• Deductible Options
• Coinsurance Options
• Physician Office Visits
• Outpatient X-Ray
• Preventive Care
• Mammography, Pap Smear and PSA Testing
• Inpatient Hospital, Surgical Fees, Outpatient Surgery and Other Inpatient/Outpatient Fees
• Emergency Room Fees
