Basic Life Insurance

Amounts may be based on coverage level, job level or a multiple of the participant's salary.

Accidental Death and Dismemberment (AD&D)

Provides coverage for loss of life or injuries sustained within 90 days from the date of the accident.

Dependent Life

Offers security for participant's families.

Key features include:

• Accelerated Benefit - applies to a terminal condition when life expectancy does not exceed one year.
• Waiver of Premium - for eligible employees totally disabled before age 60, until age 65.
• Interest Bearing Account - holds benefit payments.
• AD&D Seat Belt Benefit - pays additional 10% of the AD&D benefit (up to $10,000) if a person dies while wearing a properly fastened seat belt.
• Life Conversion - allows employees to convert to an individual life insurance policy when losing coverage under a group plan under certain circumstances.
