Our Medicare Complete products are available to persons enrolled in Medicare Parts A and B and living in the service area. You must not have end-stage renal disease, unless certain exceptions apply. You must continue to pay your Part B (and if applicable Part A) premium.

In order to receive coverage for most services with Medicare Complete, you must use contracted physicians and providers. This requirement does not apply to emergency or urgently needed services and out-of-area renal dialysis. To receive maximum benefit from Medicare Complete Choice, you must use contracted physicians and providers, except for emergency and ambulance services. If you receive care from non-contracted physicians or providers, a lesser benefit may apply, or in some cases, there is no coverage. Restrictions and exclusions apply to each category of benefits.

UnitedHealthcare of North Carolina, Inc., UnitedHealthcare of Ohio Inc., UnitedHealthcare of New England Inc., UnitedHealthcare of Florida Inc., UnitedHealthcare of New York, Inc., UnitedHealthcare of Alabama, Inc., UnitedHealthcare of the Midlands, Inc., UnitedHealthcare of the Midwest, Inc., UnitedHealthcare of Wisconsin, Inc., United HealthCare Insurance Company or United HealthCare Insurance Company of New York, Medicare + Choice organizations with a Medicare contract.
